Family Health Teams play a prominent role in Ontario’s Health Links
Summerville FHT is pleased to be one of only seven Family Health Teams who will be a Coordinating Partner in the new Health Links initiative. Health Links brings together health care providers to better and more quickly coordinate care for high-need patients.
