As a patient of Summerville FHT you will have the support of a multidisciplinary team consisting of physicians, nurses, nurse practitioners, social workers, dietitians, pharmacists and health promotion specialists. 

We also offer numerous Chronic Disease Management programs that aim to educate our patients and allow them to actively participate in managing their health.

Among other benefits, Summerville FHT provides patients with after-hours care through the Telephone Health Advisory Service (THAS). This service offers after-hours telephone health advice and, when appropriate, access to the Summerville FHT on-call physician. THAS is available outside regular clinic hours at 1-866-553-7205.
